Whenever a patient wants to change the shape of their lips, I always try to explore their thought process to understand why they are asking for this.  Historically this would be a red flag in patient selection, however in today’s society with access to social media,  are generally better informed and have a much better understanding of their options.   

As a provider you always want to evaluate whether the results they are seeking are anatomically possible and that expectations of process and progress are reasonable.  Education and transparency are key here.  Be honest and open with your patients, let them know exactly what to expect...how much product, cost, over what time frame, and possible worst & best case scenarios.  

In this case we injected 2 syringes of Juvéderm Ultra Plus, over 2 visits 3 months apart.  This after photo was taken 6 months after the 1st treatment.

Botox Is Finally Approved by FDA for Use in the Forehead Area

With new uses of Botox Cosmetic being explored every day (like a cure for depression, a temporary fix for a gummy smile and the unfortunately named Scrotox), it’s hard to believe the drug isn’t already approved by the U.S. Food and Drug Administration for all of its many indications. Even though the substance has been around since the '50s and '60s, it was only in 2002 that Botox Cosmetic received approval for treating frown lines and facial wrinkles (and it was just approved for use on crow’s-feet in 2013). If it seems like Botox Cosmetic has been treating forehead wrinkles for a very long time, that’s because it has. Doctors have been using the drug to treat forehead lines “off-label,” meaning outside of its FDA-approved use, for decades. Will I need time to recover?


After KYBELLA® treatment, you will likely have some swelling, bruising, or numbness under your chin. Be sure to inform your healthcare professional if you develop an uneven smile, facial muscle weakness, or difficulty swallowing.

What are the possible side effects of KYBELLA®?

KYBELLA® can cause serious side effects, including nerve injury in the jaw that can temporarily cause an uneven smile or facial muscle weakness (experienced by 4 out of 100 people in clinical studies) and trouble swallowing (experienced by 2 out of 100 people in clinical studies). In clinical studies, nerve injury in the jaw resolved on its own in a median of 44 days (range of 1 to 298 days), and trouble swallowing resolved on its own in a median of 3 days (range of 1 to 81 days).

The most common side effects occur in the treatment area and include:

  • Swelling
  • Bruising
  • Pain
  • Numbness
  • Redness
  • Areas of hardness

Side effects are usually temporary and often resolve on their own.

What should I tell my provider before treatment?


Before receiving KYBELLA®, tell your healthcare provider about all of your medical conditions, including if you:

  • Have had or plan to have surgery on your face, neck, or chin
  • Have had cosmetic treatments on your face, neck, or chin
  • Have had or have medical conditions in or near the neck area
  • Have had or have trouble swallowing
  • Have bleeding problems
  • Are pregnant or plan to become pregnant (it is not known if KYBELLA®will harm your unborn baby)
  • Are breastfeeding or plan to breastfeed (it is not known if KYBELLA®passes into your breast milk; talk to your healthcare provider about the best way to feed your baby if you receive KYBELLA®)
  • Are taking any medications, especially blood thinners

Danielle Smith Miami Aesthetic Nurse practitioner treats all of her patients with the utmost respect and consideration for their needs and wants above all else.  Danielle Smith NP takes a unique approach in providing her patients care by conducting a thorough Aesthetic Assessment of her patients prior to recommending any services.  Some patients may come in with a request for botox because they want to eliminate certain wrinkles, however the Aesthetic Assessment may reveal that the patient actually requires filler or may just need some slight modification to their skin care regime.   As a new patient you can expect to spend at least 30 minutes with Danielle Smith NP during the review of your

Aesthetic Assessment.  At this time you will see how Danielle will become not only your provider but your EDUCATOR, your ADVOCATE, and your COLLABORATOR.  

Danielle Smith NP-C most often accomplishes her results with a combination approach to attack the problem from multiple vantage points.  Danielle Smith NP-C, treatment plans usually involve a consortium of Dermal Fillers, Botox and/or Skin Care Products.  Each patient's needs are extremely different, which is why Danielle Smith NP takes the time to specifically assess each patient to determine your specific needs and expectations.  Your aesthetic goals will determine the course of your treatment plan.  The amount of volume loss that you have experienced will determine the amount of volume replacement that you require.   Danielle Smith NP will work with you to develop a plan that works best for you.  Plans can either include a single procedure or treatment, or a series of small volume additions.  Danielle Smith NP-C will also address your skin care regime with you to optimize your skin care, prevent further aging and being to restore any skin damage.

 

THE PATIENT INTERVIEW

The first question Danielle Smith NP will ask when you come in for your complementary consultation is 

"When you look in the mirror, what bothers you the most?"

When you Danielle Smith NP-c comes to see you for your Bedside Botox appointment, make sure you have thought very carefully about this question and your answer.  If you cannot pin-point one thing, that is okay, a general answer will work as well.  The point is to give Danielle insight into what bothers you the most.  The second question Danielle Smith NP-C will ask you is, 

"What would you consider a successful result?" 

In other words, what do you want to accomplish? Do you want to look better than you ever have, or do you want to just erase a few years off your features?  The difference is between changing your topography and between enhancing/ rejuvenating what you already have.  The best example of this is with a Lip Augmentation (or using dermal filler in the lip/mouth region).  Lip Augmentations can be used to highlight the borders of your lips giving you more definition and helping to soften wrinkles around the mouth as well as helping to turn the corners of your mouth upward (enhancing/rejuvenating).  Or Lip Augmentations can be used to provide a new fuller, plumper look that you have never had before (changing topography).  If you are unsure, that is okay as well, Danielle Smith NP-C will assess your wants, your needs, and your goals and help you develop and design a treatment plan that works best for you.  

 

MEDICAL BACKGROUND

Danielle Smith NP will have you fill out a Past Medical History form upon arrival prior to any procedures or prep.  During the consultation she will also review this with you to make sure she has a complete medical picture prior to recommending any treatments or products. 

 

SKIN CARE PRACTICES

As an Aesthetic Nurse Practitioner, Danielle Smith NP-C will want to discuss your current skin care products and regime.  Be sure to take note of your current products so she can discuss them with you to ensure they are the best possible products for your skin care needs.  You will want to mention how often you receive other treatments as well, including facials, peels, masks or other cosmetic procedures and how they worked for you.  

 

THE CONSULT

After Danielle Smith NP has determined your wants, your needs, and your goals, she with preform the Aesthetic Assessment by evaluating your natural/ baseline Aesthetic features.  This will include your: 

·       Natural topography 

·       Physical signs of aging: Skin Changes, Fat Pad Loss, Bone Loss

·       Skin : Skin Type, Color, Texture, Elasticity 

·       Skin damage: Scarring, Sun Spots, Aging Spots, Acne

·       Symmetry 

·       Wrinkles in rest and Animation 

·       Volume loss: depth of skin folds, hallowed areas

 

At this time Danielle Smith NP-C will also assess your Pain tolerance.  Some patients are much more sensitive than others.  If you have had procedures before, she will ask you how you handled those, if you experienced discomfort, how you were able to manage it, and if you required extra anesthetic.  The discomfort associated with these procedures is extremely short.  After the procedures you may have some mild tenderness in the area, but nothing will require pain medication to relieve the symptoms.  At most Tylenol may be needed for the first day or so.  The purpose of assessing your pain tolerance, is to gauge how much local or topical anesthetic you may require prior to the procedure.  

 

LIFESTYLE CONSIDERATIONS

Danielle Smith NP is very considerate of how these procedures may affect your plans.  There is little to no down-time associated with these procedures.  However, you may experience some tenderness, redness, bruising and swelling in the area for a few hours to days depending on the procedure and your skin type.  If you are wanting to get procedures done so that you are ready for a specific event, make sure you schedule your appointment at least 2 weeks before that event to ensure that you will look your best.   Don't forget it takes botox anywhere from 3-7days to take full effect.  Danielle Smith NP-C prefers to see her patients for follow ups, 2 weeks after procedures.  At this point everything will have settled and allows her to see your true results and determine if additional procedures should be layered on at this point.  

 

PLAN DEVELOPMENT

Toward the conclusion of your Aesthetic Consult, Danielle Smith NP-C will sit with you and explain the results of her Aesthetic Assessment.  She will identify areas of concern and how best to approach both a corrective and preventative treatment plan.  Danielle’s plans can range from something as simple as altering your skin care regime, to a few injections of botox once or twice a year, to a series of treatments with filler to recontour and replenish volume loss.  Danielle Smith NP will discuss the plan with you and the two of you will collaborate to determine how to structure the best plan to best meet all of your needs.   This includes your personal considerations such as financial abilities, sensitivity, time schedule, current skin care products, and so on.  She will then keep this plan in your chart to refer back to on each visit.  At each visit, Danielle will revisit your plan and make adjustments accordingly.  You will also receive a generalized outline of the plan with specific instructions and tips related to each aspect of the plan via email after your appointment.  This will become your tailor made Anti-Aging guideline, so follow it closely.  If at anytime you have questions or concerns about the plan, the products, or the procedures feel free to email Danielle directly at danielle@bedsidebotox.com

 

PHOTOS

Once a plan has been decided on, Danielle Smith NP-C will take your "before" pictures to establish and document your baseline.  These pictures will be kept in your chart for future reference.  Each time Danielle Smith NP-C comes to see you, we will take your photos.  This allows us to tract your progress through your regeneration.  The quality that Danielle Smith NP is so highly acclaimed for is not just her honest and holistic approach, but also her mastery for drastic but subtle changes.  Often times patients say their friends comment on how fantastic they look, but that they cannot seem to identify what the change is.   You too may find that you can't quite put your finger on a single change but rather on an overall enhancement of your natural features, noting that you look amazingly refreshed and rejuvenated.

NOSE RE-CONTOURING
Nose. Cheeks. Lips. These three areas are the easiest way to achieve that defined chiseled look.  Nose re-contouring is a cosmetic procedure that attempts to reshape the person’s facial topography into a more visually aesthetic form.  Danielle Smith NP uses soft tissue fillers such as Juvederm and Restaylne to enhance the natural contour of your nose.  Nose Re-contouring is designed for individuals who are unhappy with the appearance of their nose.  In most cases of nose deformity or septal deviation issues a Rhinoplasty is required.  However if your dissatisfaction is related to the definition of the dorsum (flat bridge) of your nose, you may be a candidate for Nose Re-contouring.  


Often times we can experience a lack-luster appearance to our face, especially with our nose as the center focus.  Make up artisit have recently mastered altering this perception by using contouring make to highlight the nasal dorum and create defining shadows on the sides.  For the right patient this can be accomplished with dermal filler.  Danielle Smith NP can help bring that definition back to your face through nose contouring.  Danielle likes to pair this procedure with Lip Highlighting, and Cheek Contouring for the optimal results.

 

 

WHO IS A GOOD CANDIDATE?

POST RHINOPLASTY DISSATISFACTION

Danielle Smith NP has also had huge success with this procedure in treating patients who have already had a Rhinoplasty but have some small corrections that they would like to make.  The most common areas needing correction are the tip of the nose and the sides of the dorsum.  Again, not everyone is a candidate, but for those who are having the option of a 10min simple procedure rather than a second surgery is a huge relief.  
The main objective of cheek contouring is provide definition to your cheek bones to enhance and pronounce your natural features.  In addition, to giving you the toned- definition you have always wanted, this procedure can have a drastic affect on reducing sagging skin on your lower face.  Both younger patients looking for more definition and patients looking to reduce lower face skin folds can hugely benefit from this procedure.  If this is a procedure you would like to consider, Danielle Smith NP would be more than happy to meet you for a complementary consult (in person or via photos in email) to help determine your specific needs.  Please do not hesitate to find out if you may be a good candidate for this procedure. 

TREATMENT OPTIONS

Nose Re-Contouring by Danielle Smith NP is most often accomplished with Dermal Fillers such as Radiesse, Restylane and Belotero.  Radiesse has a stronger and firmer consistency, making it a great choice of re-defining structure.  Belotero and Restylane are slightly thinner fillers, offering better control over finer precision.  Danielle Smith NP’s nose re-contouring or re-shaping can sometime involve a combination of products to accomplish the best results possible.  Either way this a precise procedure and not one of quantity.  The amount of product required is minimal but enchancing areas around the nose as well may also be required for optimal results.  Your specific needs and expectations will determine the course of your treatment plan. Danielle Smith NP will work with you to develop a plan that works best for you.  Plans can either include a single procedure or treatment, or a series of small volume additions.  Danielle Smith NP will also address your skin care regime with you to optimize your skin care, prevent further aging and being to restore any skin damage.  

PRE PROCEDURE INSTRUCTIONS

If you are considering Hand Rejuvenation by Danielle Smith NP of Miami, Florida it can be completed at the time of your consultation.  To ensure you are ready for your appointment with Danielle Smith NP, make sure you have not consumed any Aspirin containing products prior to the procedure.  If you have to take Aspirin due to a medical condition do not stop taking it until you speak with Danielle Smith NP first.  Avoid drinking alcohol, especially red wine, 1 week prior to your procedure to help minimize any bruising.  You should bring a list of your current medications to your appointment and be prepared to provide a brief overview of your past medical history.  If you have a specific request regarding your chest/ cleavage rejuvenation, be ready to address these (ie, fine lines, sagging skin, deep folds, sun spots, or discoloration).  At the time of your appointment Danielle will discuss your treatment options with you to best accomplish your desired results while maintaining an overall aesthetic facial balance.  

POST PROCEDURE INSTRUCTIONS

Immediately after the procedure you can expect to have some mild tenderness and swelling to your neck/chest.   The tenderness and swelling will subside in a few days.  Patients who are considered easy bruisers or who take blood thinners may experience some mild bruising, these too will resolve on their own in a few days.  To speed up your recovery you can take Arnica supplements right before and for a few days after the surgery.  Danielle Smith NP has done extensive research into the affects of Arnica on the reduction of post procedure bruising, swelling, and pain and offers supplements for purchase on site if you forget to pick some up.  Danielle Smith NP recommends that you avoid touching the back of your neck/chest or manipulating it as little as possible.  You want to avoid sleeping on your side or wearing push-up bras for at least a week.  You will need to sleep lying on your back.  Avoid strenuous exercise for the rest of the day after the procedure.  You can resume your normal activities the next day, but to ensure the best results continue to avoid excessive manipulation of the neck and chest for about a week.  It is okay to use make up, moisturizer or sunscreen over that area, but try not to be too aggressive in your application.  Hylauronic Acid is a hydrophilic substance and will pull water into it, considering this it is important to keep your skin moisturized after the procedure.  You will also notice that when you eat excessively salty foods you may experience mild swelling, this is also related to the hydrophilic properties of the hyaluronic acid.  

FOLLOW UP EXPECTATIONS

Danielle Smith NP has all of her patients follow up in 2 weeks to evaluate your results and ensure that you are completely satisfied with the procedure and to take your post procedure photos.  Should touch ups be required they would be preformed at this time.

Q & A on Latisse

 1.       Putting mascara on and taking it off has got to be pretty rough on your lashes, right? 

It can definitely be rough on your lashes and cause damage. 

Some tips…

1)    Use gentle, ophthalmologist tested, and oil-free make-up removers such as Clinique Take the Day Off. It dissolves long wearing even waterproof eye make-up. It is safe to use around the eyes, and appropriate for contact lens wearers. 

a.     Another brand to try: They’re Real Remover, by Benefit. If you've ever worn their mascara, even though it isn't waterproof, it can still be a pain to remove. This formula is also safe for around the eyes, but it is a cream version, for all those who prefer not to use the liquid sister of eye make-up remover.

b.     Whichever you choose, you definitely want to use cotton rounds,

2)    And for heaven’s sake ladies, be gentle! Swipe softly in a downward stroke in order to minimize pulling out or damaging your eyelashes.

3)    You will the want to follow with a cleanser that is safe to use on and around the eyes. Try Caudalie Gentle Cleansing Milk which is gentle enough for around the eye area and will help facilitate further breakdown of eyeliner and mascara.

4)    Also if you can avoid waterproof mascara, that would be helpful.

2. There’s so much we can do to nourish our skin, but what are some things we can do for our eyelashes?

There are several lash conditioners on the market and if you think about it, lashes are hair follicles that we often neglect.  A few favorites include Chanel Beaute des Cils and Dior Diorshow Maximizer. Both can be worn as a primer under your mascara for added thickness, or what is even cooler is that they can be worn alone before bed due to their conditioning properties. Both contain a conditioning formula that fortifies and hydrates, while enhancing lash volume, density, and color.

Now if you want to grow your lashes or increase the volume, density, color and length….that will take more than a conditioner.  The best product on the market for proven lash growth is Latisse®.

3.       What are many women doing perhaps unknowingly that can damage their lashes?

Biggest mistake for eyelashes, besides aggressively rubbing the eye area, is false eyelashes and extensions. Overuse of both of these could result in ripping out the eyelashes themselves, or causing damage to the follicle which leads to them not growing back in.  The glue that is used for the application of false lashes can be the most damaging of all.  Its designed to stick to your lashes causing you to pull them out when removing them.  Steer clear if you can. 

4. Are there products (whether natural remedies or beauty store buys) that really work?

Latisse! Latisse! Latisse!

Latisse ® is an FDA approved prescription medication produced by Allergan that is approved to and indicated to grow eyelashes.  Clinical trials showed that Latisse provided patients with 106% fuller, 25% longer, and 18% darker lashes. 

5. Can you please list all of the steps (in detail) to complete the ultimate lash-care guide (including any relevant products)?

AM Routine

1)    Wash Face Gently around eyes

2)    Apply Lash conditioner such as Chanel Beaute des Cils and Dior Diorshow Maximizer

3)    Apply makeup as usual

PM Routine

1) Gently remove eye make-up with a gently with a gentle eye make-up remover & cleanse face.

-       Take a cotton round/pad and apply the eye make-up remover of choice. Gently stroke in a downward motion. Do not rub to towel dry, lightly pat with towel.

2)    Remove contacts if this applies.

3)    Apply one drop of Latisse via brush applicator to the eyelash margin on each eye. Do not put the product directly in your eye and do not apply it to the lower lids.   Blot away any excess. Dispose of the applicator and use a fresh one for each eye.

You will start seeing results after 4 weeks of consistent use with Latisse and up to 16 weeks for full results.

Q & A on botox & filler

1. What's the different between Botox and fillers? 

BOTOX® Cosmetic is designed to reduce the repetitive muscle contractions that cause frown lines, crow’s feet, and forehead wrinkles.  Lines and wrinkles caused by active movements or expressions are what we call dynamic wrinkles.

As the BOTOX® Cosmetic takes effect (about 5 to 7 days) you will notice a visible smoothing of your crow’s feet lines and frown lines between your brows.

Dermal filler is an injectable soft gel that acts as a volumizer, providing a subtle smoothing, plumping, volumizing effect as well as lifting areas of moderate to deeper folds or volume loss, such as smile lines, marionette lines, down turning smiles (oral commissures), lips and cheeks. 

2. When should someone start doing botox?

I will start by saying, everyone is different.  There are a lot of factors that play into facial aging and premature aging, including sun exposure, environment, diet, hydration, skin care and genetics.

The best time to start is when you are looking in a mirror and you make an expression to see the lines, then relax.  Are the lines still there?  Even faintly, can you see an impression of it?  That will only continue to increase with continue use and exposure.  It is much easier to do BOTOX® Cosmetic once a year on a preventative front, rather than coming in a few years down the road and having to do much more to correct.  Just like a healthy diet and exercise routine and lifestyle maintenance, it is never too late to start but the sooner the better the results will be.

3. What are the benefits of botox?

BOTOX® Cosmetic have some amazing benefits.  For aesthetic purposes BOTOX® Cosmetic can smooth out facial lines and wrinkles resulting in a younger, fresher-looking face.  When used on the frown lines which are the lines between the eyebrows, it softens them up so much resulting in a less angry look to the face.  When used on smile lines often referred to as "crows feet" it gives a slight lift or pulled look creating a younger face minus those deep lines and wrinkles you see every time you smile.  

We also use BOTOX® Cosmetic and Dysport for Hyperhydrosis (excessive underarm sweating). 

I personally undergo this treatment and it is not painful. I do not even use numbing cream. The effects are amazing.  I have nearly a 100% reduction in underarm sweating and am able to wear some of my favorite silk tops, even in the Miami summer!  The results last for just over 6 months.

BOTOX® Cosmetic to treat hyperhydrosis is a great option for those who don't want sweat stains or don't like using deodorant.  It is a very popular treatment all year round in Miami due to the high humidity and the big social scene and designer outfits.  Celebrities often go for Hyperhydrosis treatments before the red carpet events.

4. How can someone avoid that "frozen" look with botox?

Go light - if you go light though, expect it not to hold as long and may require you to come in more frequently

Or rotate the areas - don’t do all 3 at once

Also, some people like the frozen look – they prefer the stoic, poised and unnerved appearance that it can provide.  I find this more in people who are very emotionally expressive.  In the sense there is little buffer or filter between the thoughts in their head and the expression on their face.  They appreciate the veil that botox can provide.

At smith & popov we are very conservative and always prefer to provide a natural look to the face.  We focus on smoothing out the wrinkles but allow movement so it doesn't look like you have had botox.  It simply looks as if you look youthful and refreshed.  We do get some requests from our clients who want to have a "frozen" tighter and smoother look and some benefits include a more awake wide eyed look and as well as a subtle brow lift appearance.  After we spend time discussing the options we always work together to get the results our clients want without going too extreme.

5. Fillers...what do they do exactly?

Simply put, fillers fill.  They add volume where volume needs to be added, either to restore or enhance.

Volume is super important as we age because we tend to lose it in our face resulting in a hollow, drawn and droopy look.  By using dermal filler injectables you can really build the face back up to look youthful and plumped.  Aside from age prevention, fillers are amazing for those who do not have the features that they want.  We can create those features for them.  Whether it is a full, plump lip or high contoured cheekbones.  We can add volume to various areas of the face to fix almost any superficial imperfection.

6. What are your dos and dont's of fillers, especially at a young age (25-30)

In today's media and with reality shows glamourizing botox and fillers on younger girls who happen to look really good and have great results the do's are to go slow and spend time talking to the specialist to make sure that the look you are requesting is right for you.  So just because your favorite celebrity has va va voom lips it may not necessarily be the right look for you.  Be realistic with your goals and what you want to achieve.

Do: Educate yourself – this is something that we pride ourselves on at smith & popov.  We welcome questions and consultations.

Do: Keep it natural. Do things in stages and not too much at once. If someone is advising you to do more than you are comfortable with in the first treatment, you may want to get a second opinion or do more homework.

Do: Your homework on your provider.  The least or most expensive deal in town is not necessarily the best. 

In Florida you want to look for providers that are Nurse Practitioners (ARNP, NP), Physician Assistant’s (PA), Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), or Dentist (DMD). 

Not all botox is created equal. 

Do: Make sure you are getting US Allergan BOTOX® Cosmetic from a verified provider.  You can check https://www.botoxcosmetic.com to make sure your provider is registered under “find a provider.

Do: Follow provider pre and post treatment instructions and follow up recommendations.

Do: Maintain your investment – take care of yourself and your skin

- use your sunscreen, wash your face, remove your make up at night, eat a balanced healthy diet, workout regularly, drinks lots of water, don’t smoke, and limit excessive alcohol.

7. How often do these procedures need to be touched up?

When first starting with a patient, I tell them to expect to come in for a touch up.  It takes building a relationship with the patient to get to know what they like and to see how the products work for them.  I much prefer to be conservative to start and then add on whatever additional is needed for a touch up.   After that, it's very infrequent.

7. Add On - What does a maintenance plan look like?

It depends on each patient and how far along they are in the aging cycle. 

If you have moderate to severe wrinkles I would recommend staying on top of your BOTOX® Cosmetic cycle with consistency at every 3 to 4 months.

When you have deeper lines that we are working to correct, I advise my patients not to wait for the wrinkles come back, but to come in when they start seeing the movement return.  Don’t wait for the lines to fully form again. That’s how we stay ahead of them. 

If you are more of a preventative case, you can get away with doing BOTOX® Cosmetic once every 6 months to a year.

8. Is there a way to get these done without looking "fake" going for a more natural look?

Absolutely, it is all about finding the right provider who values that natural look as well.  It is always best to start low and go slow.  Dip your toe in, try it out, add it on slowly.  You can always ask to see your provider’s “before and after” photos to get a sense of their injecting style.

Keep it to 1 to 2 syringes per treatment with filler then go back after the product has settled.
